Communication
We do not have a front office, therefore the best way to communicate is via the client and patient portal. This will be set up prior to your initial visit. We also ask that you do not send any personal information via email. We have a Hippa Compliant portal that is the way we send and collect your health information and fax documentation to other health professionals regarding your care. The portal is where clients and patients can schedule, message their provider or trainer/coach, access home exercise programs, conduct telehealth and virtual visits, make payments, and access superbills for insurance submission if appropriate. I can respond faster and more easily between clients and patients via messaging through the portal, than by phone if you have a concern. On occasion, I may have an independent contractor, who has signed a document verifying she will abide by all the HIPPA rules and regulations. This independent contractor may be responding to emails and phone calls. This will only be the case if I am out of the office for an extended period of time or the volume of calls and emails are too many to respond to in a timely manner while also treating patients.
You can call the office number at 443-481-9239. Voicemails will only be checked between 9:30-2:30pm on the days the office is open, and I will make every effort to respond to all calls made during Monday through Friday within a 24-48 hour period. The office is closed on Weekends.
In an Emergency Situation, physical therapy is not appropriate and clients should go to the nearest emergency room and contact their primary care provider.
Once you are an established client or patient at Form and Motion will be able to send documents and communicate via the portal.
